The information request form or application in free format shall be signed by the person requesting information – a natural person or a representative of the legal subject. No authorization required.

Additional information:

• Information requests must include the name, date and number of the requested document.
• When requesting documents from the non-public domain or documents from the record-keeping, the requester shall state in the form or application the reasons and the purpose of the use. In case of lack of justification and purpose, restricted information (e.g. personal identification numbers, residential address) shall be obscured.
