THE application form EEIG1 is to be signed by all the founders or the application shall be signed by any other person to whom it has been entrusted by the founding members (a notarised mandate to be submitted).

The application must contain:

• Group Name
• The given name, surname, personal identity number of each member of the group (if the person does not have a personal identity number - date of birth, number of the identity document and date of issue, State and institution which issued the document) and the main place of performance of the activities, but for the legal person - name (firm), legal form, place of registration, registration number, legal address and the location of its management or, if a legal person does not need a registered office in accordance with the laws and regulations of another Member State of the European Union, only the location of its management shall be indicated
• Legal address of the group
• Group Creation Goal
• If the group is established for a specified period, the time for which it was established
• Name, surname, personal identity number of each head of the group (if the person does not have a personal identity number - date of birth, number of identity document and date of issue, State and institution which issued the document, as well as indication whether the group's leaders may represent the group individually or jointly

If necessary, the boxes of the form may be copied.
