The application form KR18 shall be signed by the Management Board. If the member of the Council resigned himself or herself, the application shall be signed by him or the Management Board.

Additional information:

• when making changes to the composition of the Board, the application form in section 3.2 of KR18 shall include all the current composition of the Board, including those members of the Council for which no changes are made.

If necessary, the boxes of the form may be copied.
