The application form KR13 shall be signed by the Management Board.

Other information:

• the application shall specify the time period for the application of creditors' claims, as well as the address at which creditors may apply for their claims;
• the deadline for the application of creditors is one month if a longer deadline for the application of creditors is not specified in the meeting decision of the members or in the court adjudication regarding the termination of the activity of the SIA;
• the address of the place of application for claims of creditors must coincide with a specific address entered in the State Address Register.

If necessary, the boxes of the form may be copied.
