The application form KR5 shall be signed by an individual merchant.

The application must contain:

• The application shall indicate the given name, surname, personal identity number (if the person does not have the personal identity number of the Republic of Latvia - the date of birth, the number of the identification document and the date of issue, the State and institution which issued the document and the address in which it is to be reached) and the amount of the rights of representation.
• Where a procurist has the right to alienate, pledge or encroach property rights, it shall be specified in the application
• Several persons may be granted joint procurations at the same time. In such a case, the joint prosecutor shall be issued to at least two persons together, who have only common rights of representation.

If necessary, the boxes of the form may be copied.
