The notification of participation shall be signed by the Management Board.

The notification shall specify:

• the given name, surname, personal identity number of the shareholder - the natural person (if the person does not have a personal identity number - the date of birth, the number of the identity document and the date of issue, the State and institution which issued the document);
• the name and registration number of the shareholder, the legal person and the partnership;
• the total number of shares owned by the shareholder and the voting rights associated with that number.
