The application form PR-1 shall be signed by the legal or contractual representatives of a foreign merchant or organisation.

The application shall contain:

• Name, legal address and registration number of foreign merchant or organisation (if any)
• Country of which the foreign merchant or organisation is resident
• Basic activities of a foreign merchant or organisation
• Name, surname, personal identity number of the foreign merchant or representative of the organisation (if the person does not have a personal identity number, - date of birth, number of the identity document and date of issue, State and institution which issued the document)
• Legal address of the mission
• Duration of action (if specified)

If necessary, the boxes of the form may be copied.
