The application form PR-1 shall be signed by legal or contractual representatives of the foreign merchant or organisation.

The application shall state:

• Name, legal address and registration number (if any) of the foreign merchant or organisation
• State of which a foreign merchant or organisation is a resident
• Basic activities of a foreign merchant or organisation
• Given name, surname, personal identity number of a foreign merchant or representative of an organisation (if the person does not have a personal identity number - date of birth, number and date of issue of the personal identification document, country and institution that issued the document)
• Registered office of the Mission
• Duration (if established)
