The name of the foundation is indicated in the articles of association of the foundation, so that the relevant point of the articles of association should be amended when the name is changed.

Documents to be submitted:

• application form B3;
• a new version of the articles of association;
• minutes of the meeting of the Board;
• a document attesting the agreement of another administrative authority to make amendments to the articles of association (if such consent is provided for by the articles of association);
• if it is not possible to submit to the service portal, a receipt or a copy thereof, or a printout of an online bank payment, or information on the payment of the State fee, must be attached.
