The beneficial owner (PLG) is a natural person - a founder of a foundation, including a foundation, a Member of the board of directors, a Member of another management institution (if such has been established), a beneficiary or another natural person exercising control over the foundation, including the foundation.

The following shall be indicated regarding the Member of the foundation, including the foundation, the PLG (including the founder, a Member of the board of directors, another Member of the governing body (if such has been established), the beneficiary and other natural persons exercising control thereof):

• name;
• surname;
• personal identity number (if none - date, month, year of birth, number and date of issue of the personal identification document, country and institution that issued the document);
• nationality;
• the country of habitual residence;
• way of exercising control over a legal person:
  • through legal entity status:
    • as founder of the foundation;
    • as a representative of the executive or administrative body;
  • as a separate person controlling (if PLG indirectly controls the founder, the executive body or the representatives of the governing body):
    • on the basis of an authorisation agreement;
    • on the basis of the business relationship;
    • on the basis of ownership;
    • through a legal formation as founder;
    • through a legal arrangement as a trustee (manager);
    • through the legal arrangement as supervisor (if any);
    • through the legal arrangement as beneficiary or category thereof;
    • as another natural person;
  • other (free text field with the ability to type an undefined type)
• information on the person through whom control is exercised:
  • for a natural person, given name, surname, personal identity number (if the person does not have a personal identity number - date, month and year of birth);
  • for a legal person (may also have a foreign legal person) - the name, registration number and legal address).

It should be noted that it is not possible to provide an explanation of all situations, as each case is individual and the board of directors of the foundation should know (clarify) whether any natural person has real control of the establishment.

The Enterprise Register shall have the right to request a document confirming the relevance of the information identifying the PLG (a notarised copy of the personal identification document, a statement from the population Register of a foreign country or other documents equivalent to the referred to documents)!
