Application form B2 shall be signed by all the founders or at least two founding members empowered to sign the application in the founding decision. When establishing a psychiatry foundation, the application shall be signed by the executor, heir or trustee of the will.

The application shall contain:

• the name;
• the registered office;
• The objectives, as well as the scope according to the classification determined by the Cabinet;
• the date on which the decision on the establishment was taken;
• the given name, surname, personal identity number of the members of the Management Board (if any, the date of birth, the number of the identity document and the date of issue, the State and institution which issued the document), indicating whether they have the right to represent the association or foundation separately or jointly;
• the term of validity of the foundation, if the foundation has been established for a period of time;
• date.

If necessary, the boxes of the form may be copied.
